Our clinic is certified by US Citizenship and Immigration Services to perform physical for patients applying for a green card or US citizenship. We are able to examine adults as well as pediatric patients and provide necessary labs and immunizations that are required by the government.
We fill out form I-693 and provide patients with a sealed envelope and copies of all documentations in an open envelope for patients review.
We speak Polish and Ukrainian, and are able to communicate in other languages with assisted technology.
